After opening the Form via the link above, please click on the "Fill a Copy" Button
Step 2:
You will be asked to sign in.
Signing in with your Google Account is the easiest option, however, if you do not have a Google (Gmail) account, you will need to create one using the other options and go through those steps to confirm.
Step 3:
After confirming your account, you will need to click on the "Fill a Copy" button, one last time and confirm that you will create a new copy of the document, for yourself.
Step 4:
After the copy is created, click the checkbox for "I agree to use electronic records and signatures" and then click on the "Let's do this!" button.
Step 5:
Fill out the form.
If you are also the Emergency Contact, you can skip that section.
Step 6:
Click on the "Sign..." area and you will be given different options for applying your signature
"Draw" is likely the easier option, which allows you to use your mouse to "draw" your signature.
Click on the "Save" or "Save as Default" button when you are done.
Step 7:
Click on the "Finalize" button once you have completed all required fields, signed, and dated the form.
You will be given the option to include a personalized message, otherwise, click "Finalize Document".
